This medical invoice template given for free helps you create professional and beautiful invoices with easy and quick. The medical invoice template can be used by any healthcare provider to bill the patient and helps professional hospitals to keep the health care records.

A medical invoice or medical receipt unleashes the information regarding the medical services at a medical organization like hospital or clinic. The free medical invoice form offered by uniformsoft.com details the medical organization name and other information, patient information,
physician name, as well as detailed service performed and medicine sold. The invoice body has 7 columns, including Date of Service, Description, Total Fee (editable within invoice), CoPay (editable within invoice), Insurance Reim (editable within invoice), Ins Contract Adjust (editable within invoice), and Balance - Patient Responsibility (auto-calculated with Excel formulas, equal to Total Fee - CoPay - Insurance Reim - Ins Contract Adjust.

The medical invoice template also includes a section for you to detail and print payment information. A group of checkboxes lets you record payment method, including 'Check', 'Visa', 'MasterCard', 'Amex', 'Discover'. Two fields at the bottom of the medical invoice format are designed for writing by hand - one for signature and another for date.

Since medical invoices are used in the health care industry by the pharmaceuticals hospitals and other health care providers, the invoice template/receipt template includes a default LOGO image with a green ' medical treatment' flag on the top-left corner.
